Level One Package:

Physician medical navigator will:

·         Perform a thorough intake consultation and determine goals.

·         Review patient's medical history, medical records, diagnostic tests including labs and imaging, medications, diagnosis, and treatment plans, as well as any additional information from your personal healthcare professionals.

·         Summarize the medical information, treatment options, and recommendations in a concise written document, to have your vital information readily available to medical professionals.

·         Research patient’s diagnosis and treatment options, and guide patient to additional information, including articles and reputable websites.

Level Two Package:

Includes services in the Level One package, plus physician medical navigator will:

·         Meet with the patient and/or family members or loved ones to review the written summary and understand the treatment recommendations and options.*

·         Decipher the medical jargon and technical information about the diagnosis to achieve greater understanding.

·          Explain the proposed treatment options, including any procedures and medications, as well as the risks, benefits, and alternatives, to aid the patient in making the best personal decision.

·         Speak with the patient’s healthcare professionals, as needed, to gain greater comprehension of the case.

·         Communicate with additional healthcare professionals, researchers, and/or institutions that may provide additional assistance.

·         Research community-based resources that may be available to the patient and/or family members.

·         Assist with making appointments with healthcare professionals you would like to meet.

·         Facilitate finding quality medical care, whether it is a first, second, or third opinion.

·         Coordinate the transfer of information amongst the members of the healthcare team.

·         Communicate with insurance companies, billing departments, and pharmacies.

·         Communicate with family members and loved ones on behalf of the patient.

·         Provide emotional support to patients, their family members, and their loved ones throughout the process, from diagnosis through healing.

Level Three Package:

Includes the services in the Level One and Level Two packages above, plus physician medical navigator will:

·         Accompany patient to healthcare appointments.*

·         Meet with patient at hospital bedside, when permissible.*

·         Meet with patient and/or family members or loved ones at patient’s home.*

·         Telephone access during an agreed upon time frame.

·         Keep track of patient’s evaluations to minimize duplicate tests and the increased cost and time associated with them.

Renal Transplant Package:

Physician medical navigator will:

·         Assist with paperwork necessary for evaluation to determine if patient is a candidate for kidney transplant.

·          Research the possibility for multiple listings and, if appropriate, assist with the necessary paperwork to determine candidacy.

·         Organize up-to-date information on current lab results for each transplant center at which patient is a candidate.
